The
Department of Pharmacy is a clinical support service of Thunder
Bay Regional Health Sciences Centre. It is responsible for
promoting health and the safe and appropriate use of drug
therapy. As part of a multidisciplinary team, the Department
of Pharmacy utilizes pharmaceutical care and drug distribution
services to provide excellence in the provision of patient
care.
The Department of Pharmacy is a clinical support service reporting to the SR. VP, Patient Care Services
Monday to Friday: 8:00a.m. - 9:00 p.m. *
Weekends/Statutory Holidays: 8:00a.m. - 5:30p.m. *
(Pharmacist available on site until 6:00 p.m.)
* On call pharmacist is available after hours
- Central medication order review
- Monitoring of drug-drug interactions
- Monitoring for allergies
- Adverse drug reaction monitoring
- Drug information
- Formulary maintenance and development, including guidelines/protocols
- Support to Pharmacy & Therapeutics Committee in terms of formulary, clinical guideline development
The Department of Pharmacy provides a therapeutic drug monitoring (pharmacokinetic dosing) program for a number of agents. These include aminoglycosides and vancomycin. A medical directive has been approved by the Medical Advisory Committee, whereby the pharmacist is allowed to order serum drug levels. Dosing of these drugs is facilitated by the availability of computerized dosing software.
Thunder Bay Regional Health Sciences Centre is the regional dialysis centre for Northwestern Ontario. The Department of Pharmacy provides a pharmacist to provide drug monitoring and counselling services for the patients in this program. In addition to this, the pharmacist provides inservice education to the staff of the Renal Services Program. For further information, please contact Sean Albanese at albanesa@tbh.net.
Thunder Bay Regional Health Sciences Centre provides oncology services, to inpatients, in partnership with the Northwestern Ontario Regional Cancer Centre. The Department of Pharmacy's involvement in the Cardiac Rehabilitation Program consists of a group talk for patients enrolled in the program. Topics covered include general information on the correct use of medications and specific information regarding cardiac-related medications. The Department of Pharmacy provides warfarin counselling and dosing services to the Outpatient Venous Thromboembolism Treatment Program.
The Department of Pharmacy will soon be piloting a program to provide clinical services to the Emergency Room. In this program, the pharmacist will have responsibility for gathering medication histories, clarifying medication orders, providing drug information, and clarifying medication orders.
