Project Leader – Patient-Centred Model of Care

Patient Care Services

 

Reporting to the V.P. & Chief Nursing Officer, you will lead the strategic development and implementation of Patient-Centred Model of Care organizationally. As a leader in healthcare innovation and change, you will perform an organizational needs assessment, provide and coordinate hospital wide education strategies, lead and facilitate interprofessional teams to adopt a Patient-Centred Model of Care philosophy, and develop strategies to improve coordination of care, teamwork and interprofessional communication skills. Education and consultation to teams on advanced practice roles, the promotion of an environment that supports research based practice is also key to your success.

 

Primary Responsibilities:


Developing and leading the strategic development of a Patient-Centred Model of Care which includes the Eight Dimensions of Care (access; respect for patients’ values and preferences; coordination of care; information, communication and education; physical comfort; emotional support, involvement of friends and family; and preparation for discharge and transitions in care) throughout the organization. Initiating a process for measuring the effectiveness of Interprofessional Patient-Centred Care by using outcome measures and performance indicators.

 

Employment Requirements:


  1. Strong leadership, presentation, organization and communication skills
  2. Knowledge and experience leading innovation and change
  3. Experience with marketing and networking
  4. Demonstrated project and time management skills
  5. Previous use of organizational assessment tools
  6. Current registration with respective Professional College
  7. Adult Education Course or experience with adult education
  8. Familiar with RNAO Best practices, Patient-Centred Model of Care, Core Competencies for Health Care Professionals
  9. Familiar with research process, data collection and interpretation
  10. Advanced computer skills including Groupwise, Word, Power Point and Excel Prrograms
