Thunder Bay Regional Health Sciences Centre is committed to
providing the best possible healthcare, and we are constantly
looking for ways to improve our services. TBRHSC regularly
surveys patients as part of our efforts to improve your experience.
You may be randomly selected from our database and receive
a survey by mail once you return home. Your input is vital!
The answers to the Frequently Asked Questions below are designed
to provide you with information about our survey. Should a
serious issue arise, please feel free to discuss it directly
with a staff member or the Manager of the department. Information

Does everyone get a survey?
Patient names are entered into a database and a random sample
is selected monthly. Only the patients selected from the database
are sent a survey.
Will TBRHSC know who filled out the survey?
The responses to the survey are anonymous.
TBRHSC does not know who responded to the survey.
What information does TBRHSC get from the survey?
TBRHSC receives a monthly and quarterly report showing how
well patients rated the service and care they received. No
patient names are included in the report.
How long will it take to answer the survey?
It will take about fifteen minutes to answer the survey.
Can someone help me fill out the survey?
Yes. A family member or friend can assist the patient. They
can even fill out the survey if they are familiar with the
care that was given.
How much of my information goes to the people sending out
the survey?
They will know your name, address and that you have had a
visit at the TBRHSC. They will not have any health information.
Am I required to answer the survey?
The choice is yours. We appreciate your comments to support
us in making ongoing improvements in care that you receive.
Why does the Health Sciences Centre do this?
Thunder Bay Regional Health Sciences Centre is committed to
providing excellent patient care. The information gathered
in the survey will direct our improvement efforts. We are
also able to compare our results with other hospitals in Ontario
and Canada. We can work with others to improve our care.
